Greater Michigan Chapter, founded in 1981, is a private, non-profit, voluntary health organization and one of over 80 chapters across the United States. The Greater Michigan Chapter headquartered in Southfield, with regional offices in Alpena, Flint, Grand Rapids, Marquette, Midland and Traverse City, covers 60 counties and makes services available to more then 138,000 individuals and their families who have Alzheimer's disease or related dementias.
Area Agency on Aging of Western Michigan
Leaves Personal Care Services became a contract provider for Area Agency on Aging of western Michigan in February of 2009.
Established in April 1974, the Area Agency on Aging of Western Michigan (AAAWM) exists to help older adults and persons with disabilities live in their community with independence and dignity. Funded with state, federal, and local dollars, AAAWM does the following:
* plans, develops, oversees, and advocates for the in-home care of older adults, especially those with the greatest social and economic needs
* contracts with providers to deliver personal care, assisted transportation, housekeeping, home chore, home delivered meal, respite care, outreach and assistance, and legal assistance services to older adults
* provides Care Management services for older adults, in which a nurse and social worker team develop in-home care plans utices available in the community
* administers the MIChoice Medicaid Waiver program which allows for in-home care of Medicaid eligible adults at risk of nursing home placement
* serves older adults in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo, and Osceola Counties
* administers the Kent County senior millage
