Job Description

Position Summary: The Team Leader assesses the needs of the Leaves client and communicates with all those who are responsible for the well-being of the client. This will/can include:  Leaves Owner/Manager, Leaves Personal Care Companion Team Member, Client, Client’s designated family member/s, Leaves Scheduler, Client’s Facility, and any agency that may provide information for the well being of the Client.

Required Knowledge, Skills, Abilities :

  1. Each Team Leader must be 21 years of age.
  2. Must be a Leaves employee for at least 6 months with a positive employee review at 3 months.
  3. Must be reliable, self-motivated and respectful of senior adults and their Team Members.
  4. Ability to maintain confidentiality.
  5. Ability to follow written and oral instructions.
  6. Ability to prepare medication set-ups for client. This means a willingness to undergo a brief training period provided by Leaves; will not make decisions regarding medication changes.
  7. Ability to perform the duties of the job for the assigned work schedule.
  8. Exceptional ability to appear on time for work and notify the appropriate individual in advance if unable to work.
  9. Good human relations skills, good judgment, and the ability to work well with others.

Compassion and sensitivity to Team Members and their family needs in relation to their ability to do the duties required for their client.

Essential Functions of the Job:

  1. Provide careful, consistent, and accurate communication with the Leaves Personal Care Companion in regards to the well being of each client. It is essential that each Leaves Team Member on the team understands their respective duties and realizes their personal responsibility for that client. Each client is uniquely different and what a Team Member may do for one client may not be the same for their current client.
  2. Maintain current, up to date client information to the Owner/Manager on a weekly basis (Contact needed the end of the week- Friday-Monday.). This may be done through personal phone call, face to face meeting, or email, whichever means is established with the Owner/Manager. Once this is established, this communication must be consistent with no exception. At the end of each month, client care notes need to be turned in to the Leaves Office. Please advise the office if the client book is in need of daily notes or timesheets.
  3. Keep good communication with the Leaves Scheduler, and remind Team Members’ to have availability schedules to the scheduler no later than the date scheduled for each month.
  4. Although each Team Member is responsible to see that her/his time sheet is in, the Team Leader is encouraged to see that this is done, no later than Wednesday at 9:00 am following the end of each pay period, to ensure the well being of the client to whom they are responsible. This will be done by placing a note in each client’s notebook as well as calling each Team Member.
  5. Report any problems, or needs for additional Team Members directly to the Owner/Manager or the Leaves Office Manager during daytime office hours.
  6. Maintain neat and professional appearance at all times: dress pants, slacks, casual professional attire in accordance with Leaves Personal Care Services. No jeans, below-waist pants allowing bare midriff or underwear to be seen, low cut or off-shoulder tops, spaghetti straps, or short skirts will be allowed.
  7. A Team Leader will exemplify above-average compliance and communication as a Leaves Team Member.
  8. Perform other duties and special projects as assigned.
  9. Please keep the office updated regarding schedule changes made between team members during the week- switches, covering for sickness, etc.
  10. Contact each team member weekly to make sure they are aware of their schedule for the following week.


 

 


Personal Care Companion

Job Description


Position Summary: This position works in cooperation with the leadership, family, and other team members of Leaves Personal Care Services, and any retirement/senior apartment facility staff. Responsible for the holistic care services of our clients, as stated in their Personal Care Service Plan.

Required Knowledge, Skills, Abilities :

  1. Experience in health and human resources a plus, but mainly a respect for any person/s who need assistance.
  2. Ongoing willingness to learn new skills.
  3. Able to effectively communicate with others and a high degree of tactfulness.
  4. High School Degree or equivalent, at least 21 years of age.
  5. Maintains professionalism in areas of attitude, communication, timeliness, attendance, attire, policies, and procedures.
  6. Able to work independently, as well as in cooperation with others.
  7. Understands or willing to work with physically or mentally impaired clients, possible having a form of dementia.
  8. Willingness to work 2 Saturdays and 2 Sundays
    a month, and holidays, on a rotation basis.
  9. Has a driver’s license, insurance, and a good driving record.

Physical Qualifications:

  1. Able to lift, bend, stoop, and pivot, if necessary for the care of the client.
  2. Able to read, hear, speak, write and communicate on the phone and in person.
  3. Cognitively able to reason and problem solve.
  4. Possess the fine motor skills as to be able to write reports, etc.
  5. Able to lift or assist an adult to transfer, either alone or with someone else.
  6. Able to perform simple household tasks, cooking, bathing, and care.

Essential Functions of the Job:

  1. Ensure compliance with company policies, standards, training, and meetings.
  2. Willingness to submit to a background check.
  3. Interact professionally with other staff, family, doctors, and agencies.
  4. Follow the personal service care plan of the client, as needed.
  5. Follow-up on time sheets and any paperwork needed for the client and/or agency.
  6. Communicate needs, concerns of client, family, staff, to Team Leader or Managing Member when needed.
  7. Perform other tasks, as requested by the company leadership.
